Guidelines for email updates

To send Dr. Tarnow email updates, please follow these guidelines:

• Please state the patient's name and date of birth.
• Use bullet points to list just the facts.
• Always list ALL medications the patient is taking, including those prescribed by other doctors.
• Rate the patient's symptoms separately according to this scale: 0=None→5=Very often. For example, if the symptom of hyperactivity is experienced frequently but depression symptoms are not present, you would list each symptom and corresponding rating separately (e.g. hyperactivity=5, depression=0).
• List all side effects currently being experienced by the patient.
• Email only one patient's update at a time. If you need to send an update for another patient, please do so in a separate email.
